PCPs are responsible for the care coordination of individuals with multiple chronic diseases including those with mental health disorders. CI wants to provide PCPs with the following services to increase access to care specialists, additional clinical decision support, and opportunities for higher reimbursement.
Regularly administering M3 screening; a mental and behavioral health evaluation tool used to identify disorders that require treatment.
Monthly engagement and consultation by counselors and psychiatrists to assess your patients’ needs and make recommendations.
Monthly engagement by health professionals to assist you with the assessment and care of those patients with multiple chronic conditions
Daily Remote Patient Monitoring to monitor your patients to identify acute changes in biometric data.
CI Care Teams will seamlessly integrate with your existing practice structure to engage your patients and return to you actionable data and recommendations based upon evidence-based best practices.
Provides oversight to the clinical team and assists with higher acuity needs
Reviews assessments and indicators from clinical decision support tools
Engages regularly with patients to ensure they are responding to assessments
